Safeguarding reference questions for care — what to ask, why it matters, what to do with the answers

The safeguarding questions in a care worker reference are not a formality — they are the primary mechanism for identifying professional conduct concerns that a DBS check cannot reveal. RefAssure makes three safeguarding questions mandatory on every care reference. From 99p, no subscription.

Safeguarding Reference Questions for Care

The safeguarding questions that matter — mandatory in every RefAssure care reference

Three questions are mandatory in every RefAssure care reference and cannot be removed from any template: suitability to work with vulnerable adults, disciplinary and safeguarding investigation history, and whether the referee would re-employ in a care role. These questions are the safeguarding evidence CQC expects — and the mechanism for identifying candidates who should not be appointed.

Safeguarding Reference Questions for Care

Suitability to work with vulnerable adults

An explicit, direct question: does the referee have any concerns about this candidate's suitability to work with vulnerable adults? This question must be answered before the reference form can be submitted. The referee's written answer is captured and highlighted in the PDF report.

Disciplinary and safeguarding investigation history

Has this candidate been subject to any disciplinary procedures, safeguarding investigations or conduct concerns? This question accesses information that a DBS check cannot — internal proceedings that were never reported to police. Mandatory on every care reference.

Would you re-employ in a care role?

Would you re-employ this candidate in a care role working with vulnerable adults? If yes — in what capacity? A referee who answers no, or qualifies their yes, is signalling a concern that must be followed up. This question is included as standard in every RefAssure care template.

All answers in the CQC-ready PDF

The referee's answers to all three safeguarding questions are captured in writing and highlighted in the CQC-compliant PDF report — filed in the recruitment record and available for inspection.

Safeguarding reference questions for care — why the questions matter as much as the process

The three safeguarding questions every care reference must include

CQC does not prescribe exact reference questions, but it does require providers to demonstrate that their safe recruitment process was appropriate to the risks of each role. For care roles, this means references must address the safeguarding risk directly. There are three questions that every care reference should include as mandatory fields: suitability to work with vulnerable adults, disciplinary and safeguarding investigation history, and whether the previous employer would re-employ the candidate in a care role.

A reference that asks about performance, attendance and reasons for leaving — but does not ask about safeguarding suitability or disciplinary history — provides very limited safeguarding evidence. It may satisfy a general employment reference requirement, but it does not satisfy CQC's safe recruitment expectations for a care role.

What to do when a safeguarding question raises a concern

When a referee indicates concerns about a candidate's suitability to work with vulnerable adults, or discloses a disciplinary or safeguarding history, the care provider must act on that information before making an appointment. This means investigating further, exploring the concern with the candidate, and making a documented decision about whether to proceed. The reference, the investigation, and the decision should all be retained in the recruitment file. RefAssure provides the written reference evidence — the investigation and decision rest with the care provider.

Safeguarding Reference Questions for Care — questions answered

Every care worker reference should include: an explicit question about suitability to work with vulnerable adults, whether the candidate has been subject to disciplinary procedures or safeguarding investigations, and whether the previous employer would re-employ the candidate in a care role. These are the core safeguarding evidence fields CQC expects under Regulation 19.

References are the only pre-employment check that can reveal professional safeguarding concerns not visible on a DBS certificate. Internal disciplinary proceedings, safeguarding investigations and conduct concerns handled by previous employers are only accessible through direct reference questions. If these questions are not asked, the information is lost.

The provider must investigate further, explore the concern with the candidate, and make a documented decision about whether to proceed with the appointment. The reference, the investigation and the decision reasoning should all be retained in the recruitment file.

Yes. Three safeguarding questions are mandatory in every RefAssure care reference template and cannot be removed: suitability to work with vulnerable adults, disciplinary and safeguarding history, and whether the referee would re-employ in a care role.
